A man named Cain (Duhamel) wakes up in a Mexican hospital with no memory of how he got there, or even who he is. He quickly finds himself fighting back against a cartel who have locked down the hospital, and scrambles to piece together his true identity as well as why the cartel seems to be targeting him…
